In the last several years, healthcare organizations have begun migrating away from primarily paper-based systems and moving toward electronic medical records and charts. UCSF MyChart is one example of this. Operated and managed by the UCSF School of Medicine,
MyChart UCSF – ucsfmychart.ucsfmedicalcenter.org allows patients seamless access to the health records of themselves and their dependents. This improved access to information allows for better overall care, as patients can keep track of their health and make changes as needed.

We live in a very busy world. Between taking the kids to their sporting events and dance practices, putting in a 40-hour workweek, and remembering to grab dinner for that night, it can be easy to let health-related matters fall by the wayside.
But having access to your family’s health records via ucsfmychart org allows you the following benefits that can save you time, money, and stress:
1. View some medical records for yourself and any dependents you are a registered proxy for.
2. Making and confirming non-emergency appointments with your healthcare provider.
3. After-hours or between appointments, communicating with your provider’s office via messages. (Note this s not to be used for emergency cases. If you are concerned you are having an emergency, call 911 or go to an Emergency Room immediately.)
4. Reviewing some past appointments and procedures.
5. Accessing lab results from prior visits and comparing them to other results for the same tests.
6. Viewing and setting up prescription refills.
7. Request a referral to another provider.
When you have an account with www ucsfmychart org, you can access this information at any time, from any computer. No more worrying about making a doctor’s appointment if your computer goes down; just borrow a laptop and log in. Or download the handy smartphone app from either the App Store (iOS) or Play Store (Android). No more wondering how you’ll make an appointment at 11PM when your provider’s office is closed; you can log into the portal and schedule your appointment easily. And don’t worry about your information being secure; unlike standard e-mail or other communications, all communication within UCSF MyChart is SSL encrypted, so no one outside of you and your provider can access your health information.
Creating an Account with UCSF MyChart:
1. Before you can create an account, you will need an activation code. You generally receive one from your healthcare provider; if you did not receive one or you misplaced it, you will have to request one from the ‘I Need an Activation Code’ link on the UCSF MyChart login page.
2. Once you have an activation code, click the ‘I Have an Activation Code’ link on the login page.
3. Enter your activation code, the last 4 digits of your Social Security number, and your date of birth, then click ‘Next’.
4. Continue following the instructions and input all required information to set up your account.
